Passport Requirements:
The Clerk of District Court is a passport acceptance facility. To apply for a passport, you will need the following:

  • Passport Application
    You can obtain an application either from this office, or you can download a form and obtain information from the U.S. Department of State.
  • Proof of Citizenship
    The proof of citizenship (birth certificate, certificate of naturalization or old passport) will be mailed to the Central Passport Agency with your passport application and returned with your passport. You will need to present one of these documents:
    • Your original or a certified copy of your birth certificate, issued by the Department of Vital Records. (Not all birth certificates are alike! Click here to see birth certificate requirements.)
    • A previous passport
    • An original Certificate of Naturalization
  • One passport photo - Photos must be 2” x 2”, full face, with a white background. Although you may take your own photo, we highly recommend you have it done professionally.
  • Driver’s license or other valid, approved government-issued identification
Passport Fees:
 
Payment of the Application Fee to US Department of State must be by check or money order only. Payment of the Acceptance Fee to District Court may be by check, money order, cash or credit card. 

Adult Passport Book
$130.00 Application Fee payable to US Department of State. Include full name and birth date on check memo line (check or money order only) AND a $35.00 Acceptance Fee payable to Clerk of District Court

Child Passport Book
$100.00 Application Fee payable to Department of State. Include full name and birth date on check memo line (check of money order only) AND a $35.00 Acceptance Fee payable to Clerk of District Court

Expedited Passport Fee: Add $60.00 for each application, include amount in check to US Department of State

Child Passport Requirements
If you are applying for a passport for a child under the age of sixteen, both parents must appear with the child to sign the passport application and provide identification. There are authorization forms available at the website listed above if one of the parents is unable to appear. Please call us for information on situations such as sole custody.
